New Patient Registration Procedure
You will need to complete a registration form which will provide useful information about yourself. To register we will require photographic proof of identification and a proof of address dated within 3 months of issue. All Jersey residents will require a valid Social Security Health Card (if applicable).
The registration form can be printed out online and brought into the practice or you can come to collect a form.
Registration may take up to 10 working days. Once reviewed you will be notified in writing or email if your registration has been accepted/ declined.
Your first appointment will be a 30 minute slot and payment is required on the day.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
